Dear Patient,
As you know already, I’ve been on maternity leave for the last 10 months. Since I’ve been off, there have been some major changes in my personal life that requires me to take a prolonged time off from full time patient care. Thus, I am heavy heartedly writing to inform you that, as a result of this situation, I will no longer be able to care for you as your Family Physician, as of January 15, 2024.
I will be available at the clinic for few days a week (part time) tentatively from November 2023 until January 15, 2024, to wrap up my practice.
Booking will open on October 30th, 2023.
*** Please DO NOT contact the front office staff regarding this letter, as they have no additional information to provide, and will be unable to assist you. Nor will they be able to make exceptions for individual patients.

I want you to continue to receive proper medical care and attention. Some suggestions on finding a provider:
Inquire with your family/friends/personal network to find a physician; someone may know of a doctor who is accepting new patients.
Contact your local Division of Family Practice who can assist you in finding a new family doctor within your city of residence. We have alerted the Patient Attachment Team at the Division of Family Practice of our situation, and they are ready to support residents.
